Privacy Notice 2018

Under the new GDPR coming into force at the end of May 2018, I am required by law to document what personal information I keep for all clients, whether they be a one off appointment or regular clients.

At the time of booking your appointment I will take your name, and contact telephone number.

At your initial face to face appointment I will take your name, address and telephone number with your consent and any medical history which you feel is relevant and that may impact on the suitability of any treatments.

All previous and future clients have a treatment record card which records all the information above plus the treatment you are receiving. These record cards are kept in a locked cabinet to which only I, Jo Fisher trading as JoJo’s Therapy Room, have access.

I am required to keep this information for legal and insurance purposes. All information is kept for a period of 7 years from initial communication after which it will be securely destroyed unless you are still a client of mine. You do have the right to see any information I keep and you can request that any data be changed or securely destroyed. If however you do request that records be destroyed I will no longer be able to carry out any treatments for you. 

I do not use the information for anything other than to contact you regarding your appointment or to discuss matters relating to your treatment.

I will not pass your personal information to any third party.

I will not use your personal information for marketing purposes.

Any information given to me via the message service on my Facebook Page, Instagram or text, is only accessible by myself, Jo Fisher. You may request that once I have written your details on your record card that I delete any message which contains your personal information. 

Any information you have given me via text message on my mobile phone is accessible only by myself and my phone is locked with a security code and fingerprint recognition. You may request that these text messages are deleted once I have written the information on your record card.
